Please make sure that all of the required
information is entered clearly and
accurately. The latest version of the Bio-Concepts
Upper Extremity
Measurement Chart is dated January 2008. The forms needed for hand
garments are available below as Adobe Acrobat Reader files. If you
do not have Adobe Acrobat Reader on your computer you will not be
able to view and print these documents. Don't worry, it's free at
the adobe website: http://www.adobe.com/.
Just follow the instructions.

It is extremely important that our
designers have complete and accurate measurements. Any marks or special instructions may
be entered on the back of the chart (when you fax your order, make sure you fax both sides
of the measuring chart). Do not use light colored ink pens or colored felt pens, some
colors do not fax well, if at all.
We are happy to modify the typical upper
extremity garment styles. Make sure you identify all desired options and modifications
directly on the measuring chart.
