and neck garment order using the Bio-Concepts Head and Neck Measurement Chart. Please make
sure that all of the required information is entered clearly and accurately. If you do not
have a reasonably clean copy of the measuring chart, call Bio-Concepts at the toll free
number and we will send you a clean set of order forms right away.
The latest version of the Bio-Concepts
Head and Neck
Measurement Chart is dated January, 2008. The forms needed for hand
garments are available below as Adobe Acrobat Reader files. If you
do not have Adobe Acrobat Reader on your computer you will not be
able to view and print these documents. Don't worry, it's free at
the adobe website: http://www.adobe.com/.
Just follow the instructions.

It is extremely important that our
designers have complete and accurate measurements. Any marks or special instructions may
be entered on the back of the chart (when you fax your order, make sure you fax both sides
of the measuring chart). Do not use light colored ink pens or colored felt pens, some
colors do not fax well, if at all.
We are happy to modify the typical head
and neck garment styles. Make sure you identify all desired options and modifications
directly on the measuring chart.
